How to Register With The Practice

PreRegistration

You can either print off the Registration form below or come into the Practice and collect a form.  A form requires to be completed for each member of the family wishing to register, no matter what age they are.

Once you have completed the form bring it to the surgery with your identification documents - adults (ie over age 16) need to provide two pieces of identification. One piece of ID needs to be photographic eg driving licence, passport and another needs to be a utility bill/bank statement etc showing your name and current address. If you have a yellow medical card please also bring it with you.  Patients from abroad may need to provide other documents depending on where they are from.

When you register you will also be asked to fill out a medical questionnaire - this can also be printed out and brought in. This is because it can take a considerable time for us to receive your medical records. You will be offered an appointment to see one of the practice nursing staff.
